INFORMATION AND CONSENT TO PERSONAL DATA TREATMENT

The Royal Thai Honorary Consulate in Venice, San Marco 1808, in person of the Consul General, Mr. Andrea Marcon, informs yourself, as per §13 of the Privacy Protection Act DLGS. 196/2003 that your personal
data handed to the writing Consulate are treated as per above mentioned act. Data treatment includes any
operation or more operations, aimed with our without electronic/automatic means about collection, extraction, comparison, usage, interconnection, blockage, communication, diffusion, cancellation and destruction
of subject’s personal data.
Purposes of such treatment, exclusively by means of paper instruments, affect the request and release
of visa type:..................................................................... (specify type of visa whether for tourism or other
purposes) for entrance into Thailand.
Bestowal of such data is not mandatory but the refusal of giving all data may result in denying visa release;
above mentioned data will not be diffused/broadcasted, will be treated only by treatment right bearer and
his own staff, for 12 months of time to be destroyed at the end of such time.
We assure that all information will be carried out under strict observance of the act regulations and will be
strictly connected with the described activity purposes.
Aiming to your consent evaluation, please consider that requested data, spontaneously given will include
personal information designed as “sensible” by the Law, such as: health status, religion, political opinions).
Some of these data may also be related to some of your relatives and your consent will be automatically
transferred to them, too.
You will be enabled to apply at the Privacy Service of the Treatment Bearer to control your data, correct, or
integrate them and/or to practice your rights as per §7 of Privacy Act D.Lgs. 196/2003, herewith attached;
you are informed that the bearer of data treatment ids the Royal Thai Honorary Consulate of Venice, San
Marco 1808.
ACT OF CONSENT
I, undersigned, declare of having duly noted the present information as per §7 of the Privacy Act D.Lgs.
196/2003, with description of the rights of the concerned individual and I expressly consent to the treatment
and communication of my personal data in the terms herewith described, with possible extension to data
of my relatives. Such consent is also extended to my personal data designed by the Law as “sensible”, of
which nature I am informed.
